The coronary sinus reducer for the treatment of refractory angina

چکیده
We were very interested to read the recently published manuscript ‘Safety and efficacy of a device to narrow the coronary sinus for the treatment of refractory angina: A single-centre real-world experience’ by Masieh Abawi et al. [1]. Some people make a mistake and compare the coronary sinus reducer (CSR) with the Beck procedure (Beck operation), but actually the Beck procedure has almost nothing in common with the CSR. In the 1940s, Dr Claude Beck described two types of coronary sinus (CS) interventions [2, 3]. The Beck I procedure consisted of the following steps: 1) abrading of both epicardium and inner pericardium. Dr Beck explained that it produces mechanical trauma, which in turn causes inflammation and intercoronary arterial channels. 2) spilling of powdered asbestos and 5% aqueous trichloracetic acid on the epicardium. Dr Beck explained that this produces chemical trauma, which in turn also results in inflammation and intercoronary arterial channels. 3) CS constriction to a diameter of 3 mm. The Beck II procedure consisted of a vascular graft between the descending aorta and the CS followed by operative external constriction of the CS ostium a few weeks later. Both procedures needed thoracot-

Coronary Sinus Reducer Stent for the Treatment of Chronic Refractory Angina Pectoris
Background Increased coronary sinus (CS) pressure can reduce myocardial ischemia by redistribution of blood from nonischemic to ischemic territories. The Coronary Sinus Reducer is a percutaneous implantable device designed to establish CS narrowing and to elevate CS pressure. In preclinical experiments, implantation of the Reducer was safe and was associated with improved ischemic parameters. I...

OBJECTIVES This study sought to evaluate the safety of the Coronary Sinus Reducer (Neovasc Medical, Inc., Or Yehuda, Israel) as a potential alternate therapy for patients with refractory angina who are not candidates for conventional revascularization procedures. BACKGROUND Increased coronary sinus (CS) pressure can reduce myocardial ischemia by redistribution of blood from nonischemic to isc...

OBJECTIVE The coronary sinus Reducer is a recently introduced device to treat patients with severe angina symptoms refractory to optimal medical therapy and not amenable for conventional revascularisation. We aimed to assess the safety and efficacy of the Reducer in a real-world cohort of patients with refractory angina. METHODS This is a single-centre retrospective registry. Patients with se...
متن کاملA phase II, sham-controlled, double-blinded study testing the safety and efficacy of the coronary sinus reducer in patients with refractory angina: study protocol for a randomized controlled trial
BACKGROUND A growing population of patients lives with severe coronary artery disease not amenable to coronary revascularization and with refractory angina despite optimal medical therapy. Percutaneous reduction of the coronary sinus is an emerging treatment for myocardial ischemia that increases coronary sinus pressure to promote a transcollateral redistribution of coronary artery in-flow from...

INTRODUCTION Alongside the remarkable progress in therapeutic strategies destined to patients with coronary artery disease (CAD), a growing number of patients who suffer from severe and diffuse CAD not amenable for revascularization, continue to experience severe debilitating angina despite optimal medical therapy. The Coronary Sinus (CS) Reducer has emerged as a novel therapeutic strategy for ...
